Palliative Care Co-ordination Summary Record Keeping Guidance

The national information standard, which specifies the core record content to support the provision of high-quality co-ordinated care at the end of life, was published 20 March 2012 and last revised in September 2015. This short publication is a brief summary of the full record keeping guidance, which was developed in consultation with health and social care professionals to support staff using palliative care co-ordination systems.
