Making it happen - No decision about me without me - The International Society of Advance Care Planning and End of Life Care Conference 2011

Several countries across the world are simultaneously moving towards the same goal of integrating advance care planning into usual practice within their health and social care structures. The UK Government Coalition’s call for "No decision about me without me" echoes this important shift in direction, as does recent legislation in other countries like Australia and the United States, that signify a pivotal part of the process of improving end of life care. This conference will explore how to make this happen.
